dc.description	Relevance of the topic: There have been a number of studies done on palliative care services that emphasize the activities and the role of medical staff, while there is very little data on the activities of a social worker in a palliative care team at the client's home. A very small number of social workers work in a palliative care team, which provides palliative care services at home. Problem of the research: what are the activities of the social worker in the palliative care team, while providing social services at the client's home. Object of the research: activities of the social worker in the palliative care team, while providing social services at the client's home. Aim of the research: to reveal the activities of the social worker in the palliative care team, while providing social services at the client's home. Tasks of the research:1.To provide a concept of palliative care. 2.To describe the activities of a social worker in a palliative care team, while providing social services at the client's home. 3.To describe peculiarities of the activities of social worker in a palliative care team.4.To reveal the activities of a social worker in a palliative care team, while providing social services at the client's home. Methodology and subjects of the research: during the qualitative study, data was collected using a semi-structured interview. 5 specialists from the palliative care team participated in the study. Results of empirical research: The analysis of the research revealed that a social worker in the palliative care team carries out activities, such as identifying needs, creating an individual plan, providing social services at the client's home and performs the roles of information provider, mediator, organizer and leader. In their activities, social workers use two models: social work with an individual and social work with a family.	en_US
dc.description.abstract	Temos aktualumas: Kalbant apie paliatyvios pagalbos paslaugas yra atlikta nemažai tyrimų, kuriuose akcentuojama medicinos personalo veikla ir vaidmuo, tuo tarpu apie socialinio darbuotojo veiklą paliatyvios pagalbos komandoje teikiančios paslaugas paciento namuose duomenų yra labai mažai. Labai nedidelė dalis socialinių darbuotojų dirba paliatyvios pagalbos komandose, teikiančiose paliatyvios pagalbos paslaugas namuose. Tyrimo problema: kokia yra socialinio darbuotojo veikla paliatyios pagalbos komandoje teikiant socialines paslaugas kliento namuose. Tyrimo objektas: socialinio darbuotojo veikla paliatyvios pagalbos komandoje teikiant socialines paslaugas kliento namuose. Tyrimo tikslas: atskleisti socialinio darbuotojo veiklą paliatyvios pagalbos komandoje teikiant socialines paslaugas kliento namuose. Tyrimo uždaviniai:1.Pateikti paliatyvios pagalbos sanpratą.2.Aprašyti socialinio darbuotojo veiklą paliatyvios pagalbos komandoje teikiant socialines paslaugas kliento namuose. 3.Nustatyti socialinio darbuotojo veiklą paliatyvios pagalbos komandoje teikiant socialines paslaugas kliento namuose. Tyrimo metodika ir tiriamieji: kokybinio tyrimo metu duomenys rinkti naudojant pusiau struktūruotą interviu. Tyrime dalyvavo 5 paliatyvios pagalbos komandos specialistai. Empirinio tyrimo rezultatai: tyrimo analizė atskleidė, kad socialinis darbuotojas paliatyvios pagalbos komandoje vykdo poreikių nustatymo, individualaus plano sudarymo, socialinių paslaugų teikimo kliento namuose veiklas ir atlieka informacijos teikėjo tarpininko, organizatoriaus, lyderio vaidmenis. Savo veikloje naudoja socialinio darbo su individu ir socialinio darbo su šeima modelius.	en_US
