Relevance of the topic. Polypharmacy is common and is usually problematic for the elderly, because using many different medicines, or because of possible inappropriate treatment, medicines can interact in the body. Drug interactions occur when the effect of one drug is modified by the concomitant or subsequent use of another drug (Sinclair ir kt., 2010). Drug interactions are a concern for patients and doctors, as the use of many medicines is becoming more common in the treatment of complex diseases. Kaunas College Faculty of Medicine pharmacy technician study programs student Dovydas Medelis submit a final thesis ‘‘Investigation of the risk factors of the phenomenon of polypharmacy in the homes of the elderly”, whose purpose is to identify the risk factors for the occurrence of polypharmacies in the homes of the elderly. Research objectives: 1. The phenomenon of polypharmacy, its causes, consequences, complications and means of reducing polypharmaceutical expression. 2. To highlight the risk factors for the appearance of polypharmacy phenomenon in the homes of the elderly. Methods of scientific literature analysis and questionnaire survey were chosen. Kaunas City X residents of social care home were interviewed. Conclusions: 1. Drug-drug interaction when one drug alters the co-administered drug activity, frequency, directly linked to the number of medicines known as polypharmacy phenomenon. Causes of emergence: Elderly, patients with more than one disease, lack of patient knowledge. Consequences and complications: increased adverse drug reactions, drug interactions, drug-disease interactions, drug incompatibilities and hospitalization. There are a number of new care models for a variety of interventions and resources to help secure patient health care. These efforts focus on improving patient safety, improving communication, and reducing hospital readmission. 2. It has been found that all respondents in social care homes suffering from at least several chronic diseases, such as x percent dementia, x percent cardiovascular disease, therefore, the daily dosage regimen does not provide the respondents with fair conditions of use. X percent of retirement home residents taking five or more medications. X percent of them engage in self-medication and use drugs that they have bought or brought in by their relatives. X percent the respondents consider the medicine not in the box, x percent releases the leaflet for the medicine. Several respondents smoke and consume alcoholic beverages. At their discretion, they use herbal teas, eat fruit or drink their juices, which can seriously disrupt the pharmacokinetics of medicines or cause synergistic effects on drug treatment. |

Baigiamajame bakalauro darbe buvo paaiškintas polifarmacijos apibrėžimas, senyvo amžiaus pacientų vaistų vartojimas, pagrindinės polifarmacijos atsiradimo priežastys, polifarmacijos komplikacijos ir pasekmės. Aprašytos intervencijos mažinančios polifarmacijos išraišką.
Nustatyti polifarmacijos reiškinio atsiradimo senelių namų gyventojams rizikos veiksniai. Išnagrinėjus mokslinę - dalykinę literatūrą bei atlikus duomenų analizę buvo pateiktos išvados.
