Darbo aktualumas. Siekiant pagerinti naujagimių išgyvenamumą bei gaivinimo
komplikacijų išvengimą, svarbu, kad medicinos personalas, o ypač slaugytojai, mokėtų tinkamai
suteikti būtinąją pagalbą naujagimiams, t. y. išmanytų naujagimių gaivinimo algoritmą.
Darbo tikslas – atskleisti slaugytojų veiklą naujagimių gaivinimo metu.
Darbo uždaviniai: 1. Aprašyti LR teisės aktais reglamentuotą slaugytojų veiklą pradiniame
naujagimių gaivinime: būklės vertinimą, stimuliavimą, įpūtimus, krūtinės ląstos paspaudimus. 2.
Atskleisti slaugytojų, dalyvavusių pradinio naujagimio gaivinimo kursuose, motyvacijos bei
informacijos apie kursus šaltinius. 3. Palyginti skirtingose sveikatos priežiūros įstaigose dirbančių
slaugytojų veiklą pradiniame naujagimio gaivinime, remiantis jų žiniomis. 4. Nustatyti slaugytojų
žinias apie epinefrino naudojimo aspektus pradiniame naujagimio gaivinime.
Darbo metodai. Tyrimui atlikti buvo gauti raštiški įstaigų vadovų sutikimai vykdyti
anoniminę anketinę apklausą, Bioetikos centro pritarimas Nr. BEC-KK(B)-03. Tyrimas buvo
vykdytas 2020-10-16 - 2020-11-16. Tyrime dalyvavo 78 slaugytojai, dirbantys X ligoninės Vaikų
skubios pagalbos skyriuje, Y ligoninės Vaikų priėmimo, skubios pagalbos ir intensyviosios terapijos
skyriuje bei X miesto GMP stotyje (atsako dažnis - 58,65 proc.).
Rezultatai. Apie penktadalis respondentų yra dalyvavę naujagimio gaivinimo kursuose. Šie
slaugytojai apie kursus sužino iš kolegų, apie pusė – iš interneto, o nedidelė dalis – iš spaudos.
Daugumą slaugytojų, dalyvavusių naujagimio gaivinimo kursuose, motyvavo noras tobulėti, žinių bei
įgūdžių atnaujinimas, noras suteikti kokybišką pagalbą naujagimiui bei siekis išgelbėti jo gyvybę. Tik
labai mažą dalį – motyvavo priedai prie atlyginimo. Didžioji dalis respondentų naujagimio gaivinimo
kursus vertina 9 balais, o penktadalis – 3 balais.
Apie pusė X GMP, X VSPS ir Y VPSPITS dirbančių slaugytojų žinojo naujagimio būklės
vertinimo ir pirmųjų gaivinimo veiksmų trukmę, visi X GMP ir Y VPSPITS, dauguma X VSPS
slaugytojų žinojo, kad naujagimio gaivinimas turi būti pradedamas nuo įpūtimų, žinojo naujagimio
gaivinimo veiksmų seką, maksimalų krūtinės ląstos paspaudimų gylį naujagimiui. Didžioji dalis
dirbančių X VSPS, Y VPSPITS ir X GMP slaugytojų nežinojo naujagimio kvėpavimo vertinimo
dažnio gaivinant naujagimį. Kiek mažiau nei pusė X VSPS ir dauguma X GMP, Y VPSPITS
slaugytojų nežinojo paspaudimų ir įpūtimų santykio, kai gaivinant nėra žinomas tikslus naujagimio
amžius. Visi X GMP, Y VPSPITS ir didžioji dalis X VSPS dirbančių slaugytojų nežinojo teisingos
rankų pozicijos atliekant krūtinės ląstos paspaudimus naujagimiui. Dauguma X GMP slaugytojų
nežinojo, kad naujagimiui krūtinės ląstos paspaudimus galima atlikti dviem pirštais.
Didžioji dalis slaugytojų žinojo tinkamą epinefrino dozę, naudojamą naujagimio gaivinimo
metu, tačiau beveik šeštadalis tyrime dalyvavusių slaugytojų švirkštų netinkamą epinefrino dozę.
Didžioji dalis tyrime dalyvavusių slaugytojų, gaivinant naujagimį, epinefriną švirkštų į virkštelės
veną, kiek mažiau nei pusė – švirkštų per intubacinį vamzdelį į trachėją, apie šeštadalis – į periferinę
naujagimio veną. Apie penktadalis slaugytojų nurodė netinkamą injekcijos vietą arba teigė nežinantys
kur ją atlikti.

Relevance of the work. In order to increase the rate of newborn survivability and avoid any
complications of a resuscitation, it is necessary that the medical personnel, especially nurses, know
how to apply first aid to newborns, i. e. know the neonatal resuscitation algorithm very well.
The aim - to reveal the activities of nurses in neonatal resuscitation.
Objectives: 1. Describe the activities of nurses in neonatal resuscitation regulated by LR
legislation: condition assessment, stimulation, insufflations, chest compressions. 2. To identify the
sources of motivation and information about the neonatal resuscitation courses of nurses who
participated. 3. To compare the activities of nurses working in different health care institutions in
neonatal resuscitation, based on their knowledge. 4. To identify the comprehension of nurses on the
usage of epinephrine in neonatal resuscitation.
Research methods. Written agreements from the managers of the institutions were received
in order to carry out an anonymous questionnaire survey, approval no. BEC-KK(B)-03 from the
Center of Bioethics. The survey was carried out on 2020-10-16 - 2020-11-16. The research involved
78 nurses working in the Children's Emergency Department of X hospital, Children’s Reception,
Emergency Care and Intensive Care Unit of Y hospital and the Ambulance Station of X city (response
frequency - 58,65%).
Results. Only about a fifth of the study participants have participated in neonatal
resuscitation courses. These nurses indicated that they learn about newborn resuscitation courses from
colleagues, about half of the respondents listed the internet and a small amount listed the press as
additional sources. The majority of the nurses who participated in neonatal resuscitation courses were
motivated by the desire to renew their knowledge and skills, provide high quality assistance to the
newborn and save his life. Only a very small part listed that they were motivated by supplements to
their salary. The majority of the respondents rate newborn resuscitation courses with 9 points, and a
fifth of them have rated these courses with 3 points.
About half of the nurses, who work in the Children's Emergency Department of X hospital
(X CED), Children’s Reception, Emergency Care and Intensive Care Unit of Y hospital (Y
CRECICU) and the Ambulance Station of X city (X AS), knew the first steps of resuscitation and
the duration of condition assessment for newborns. Every nurse, who works in X AS, Y CRECICU
and the majority of nurses from X CED knew the sequence of neonatal resuscitation steps, they knew
that neonatal resuscitation starts with rescue breaths and they also identified the maximum depth of
chest compressions. The majority of nurses, who work in X AS, X CED, Y CRECICU, did not know
the frequency of neonatal respiration during resuscitation. A slightly less than half of the nurses, who
work in X CED, and the majority of nurses, who work in X AS, Y CRECICU, did not know the ratio
of chest compressions and rescue breaths (when the date of birth is not accurate). Every nurse from
X AS, Y CRECICU and the majority from X CED, did not know the correct hand position of chest
compressions for a neonate. Almost every nurse, who works in X AS, did not know that chest
compressions to newborns can be done with two fingers.
The majority of nurses knew the correct dose of epinephrine used during neonatal
resuscitation but nearly a sixth of the nurses in the study would inject an incorrect dose of epinephrine.
The majority of nurses resuscitating a newborn would inject an epinephrine syringe to the umbilical
vein, slightly less than half would inject through the intubation tube into the trachea, about a sixth -
to the peripheral vein of the newborn and about a fifth of the nurses indicated an incorrect injection
location or stated not knowing where to inject.
